Hospice Care is our highest level of care. This is for patients who have e an illness with a prognosis of six months or less and that there isn’t anything else that can be done to improve their condition.
Our hospice patients are assigned a nurse case manager that will visit you at home to make sure you are receiving hands-on bedside care. We provide all wound care supplies, durable medical equipment, medications and things that the insurance companies often won’t cover – like a side table.
We will also assign a certified nursing assistant to help you with the activities of daily living: Personal care such as bathing, hair care, nail care, light housekeeping, and meal preparation.
Every family is assigned a social worker that helps you find additional resources and help with other things you and your family might benefit from such as ALTCS applications, DNR and living wills, and end of life planning including funeral arrangements.
Remember, even though you might have a terminal illness it doesn’t mean life is over. We are here to make sure you live the rest of your life as pain-free and comfortable as possible.
Our spiritual counselors can provide any kind of support from a listening ear to sacrament. Our counselors can also help connect you with a faith-based organization of your choice. They are here to help you – as much or as little as you want.
